Well as I recall in Wrath of Khan when Sulu was taking them to the Enterprise that Kirk was supposed to tell him that the papers were signed and that Sulu would become Captain of the Excelsior.

Shatner disagreed with this plan and totally mangled the lines to the point where editing couldn't salvage it. Shatner apparently forced the change that would keep Sulu on the crew for movies 3 and 4 and allegedly demanded he be on the crew in Trek 5.

I can't confirm about Trek 5, but am pretty confident about the Khan incident.

This also allegedly prevented Takei from getting his own series, but I never did get confirmation on that.
